Low Back Treatment Guidelines

Draping

  • Pillow under abdomen (if it is not a disk issue) and feet
  • Block pelvis and check for stability and comfort ( Ex. If R pelvis anteriorly rotated, block under R ASIS and L Greater Trochanter )

Hydrotherapy

  • Heat to low back and glutes
  • State it will be on for 5 – 10 minutes
  • Make sure heat pack is stable
  • Check in with heat visually, tactile and vocally

Specific Areas

Erectors:

  • Perform 2 neuromuscular techniques
  • Detect trigger points and state referral pattern
  • Negotiate trigger point work
  • Heat and stretch area

Multifidi:

  • Perform 2 neuromuscular techniques
  • Detect trigger points and state referral pattern
  • Negotiate trigger point work
  • Heat and stretch area

Quadratus Lumborum:

  • Perform 2 neuromuscular techniques
  • Detect trigger points and state referral pattern
  • Negotiate trigger point work
  • Heat and stretch area

Glute Max:

  • Perform 2 neuromuscular techniques
  • Detect trigger points and state referral pattern
  • Negotiate trigger point work
  • Heat and stretch area

Piriformis:

  • Perform 2 neuromuscular techniques
  • Detect trigger points and state referral pattern
  • Negotiate trigger point work
  • Heat and stretch area

Joint Mobilizations

  • Glide in correct direction
  • Oscillation of Grade 1 or Grade 2

Communication

  • Ask if client feels secure, physically check
  • Check on comfort of back and glute draping
  • Double check on comfort of glute draping
  • Check in with trigger points
  • Check in with joint mobilizations
  • Ask where pain is travelling
